Tarjeta sanitaria in Spain: how to get the health card and a doctor
The tarjeta sanitaria (health card) is your key to free public healthcare in Spain: a family doctor, specialists, discounted prescriptions, emergencies. This guide covers who's entitled (including Ukrainians under temporary protection), what you need (a NIE is not required; for temporary-protection status the right exists even without a padrón) and how to apply step by step.
Who is entitled
The right to public healthcare in Spain is broader than many people think. In particular:
Ukrainians under temporary protection — yes, free
According to official information from the Ministerio de Inclusión, Seguridad Social y Migraciones, all Ukrainians who have applied for temporary protection are entitled to free healthcare on the same terms as the local population. On arrival, initial care is provided at the CREADE centres (Madrid, Barcelona, Alicante, Málaga).
You don't need a NIE; the usual route is empadronamiento
Many people think you need a NIE for the health card. You don't. For most people the usual route to the tarjeta sanitaria is empadronamiento (registration in the municipal padrón): with the padrón you apply for the card at your nearest centro de salud.
On temporary protection? The right exists even without padrón
For Ukrainians under temporary protection the right to care arises from the status itself — the lack of empadronamiento does not deprive you of access. If registration isn't possible, the right is accredited with alternative documents or through social services (see below). Empadronamiento makes the process easier but is not an insurmountable requirement for TP holders.
If you have a padrón, start there
For those whose situation allows it, the simplest path is to register (empadronamiento) and apply with it (see our empadronamiento guide). For those without a padrón yet, the social-services/alternative- documents route works.
Application steps
The path depends on whether you already have a padrón.
Under temporary protection and still without a padrón
Don't wait for empadronamiento — you already have the right to care from your status:
- Take your TP status document — the resguardo (receipt of the temporary-protection application at the police) or the resolution/TIE.
- Go to the centro de salud in your area (or your CCAA's health unit / social services). According to the official Ucrania Urgente information, they can register you and issue the document/card based on the resguardo or the TP resolution, without a padrón certificate.
- Get the provisional document — access to a doctor right away.
- Later, once you have housing, register (empadronamiento) and update the card details if needed.
Ordinary route (with housing/padrón)
- Empadronamiento. Register in the padrón and get a recent certificate (volante/certificado).
- Find your centro de salud — the one nearest your address.
- Submit the documents at the centro de salud (or your CCAA's health unit): passport + padrón certificate.
- Get the provisional document — access to a doctor right away while the card is prepared.
- You're assigned a centro de salud and a médico de cabecera (family doctor). The permanent card is issued/sent later.
Documents
- Passport or ID document;
- Padrón certificate (volante/certificado), recent — on the ordinary route;
- for temporary protection — the TP resguardo or resolution (works without a padrón).
No padrón? Not a dead end
If registration isn't possible, the right is accredited with alternative documents: documents from any public administration, consular travel documents, a minor's school enrolment, social-services records. You can be registered through social services — no one is left without access to healthcare.
It's managed regionally: each CCAA has its own procedure
The tarjeta sanitaria is managed by each autonomous community (CCAA) separately, although it's valid throughout Spain. So the exact procedure, name and online services vary:
- in Valencia the card is called SIP (Sistema de Información Poblacional);
- in Madrid, Catalonia, Andalusia, etc. — their own services and websites.
Check the health-service website of your CCAA for the current steps and appointments.
The médico de cabecera is your main contact
The family doctor at the centro de salud is the starting point for almost everything: specialist referrals, prescriptions, certificates. You can book in person, by the centre's phone, or via your CCAA's app/website once you have the card.
